Married And No Copy of Marriage Certificate

Use this affidavit when the Student and their spouse are married but do not have a copy of their marriage certificate. This affidavit is to be signed by the Student.

Separated Or Divorced With No Dependent Children And No Formal Separation Agreement

Use this affidavit when the Student and their spouse are separated or divorced, do not have a separation agreement and have no dependent children. This affidavit is to be signed by the Student.

Separated or Divorced With Child Support

Use this affidavit when the Student and their spouse are separated or divorced and they do not have a formal written separation agreement, divorce decree or court order. The Student’s children live with the Student during the study period more than half of the time and the Student DOES RECEIVE child support from their former spouse. This affidavit is to be signed by the Student.

Separated or Divorced With No Child Support

Use this affidavit when the Student and your spouse are separated or divorced and they do not have a formal written separation agreement, divorce decree or court order. The Student’s children live with the Student during the study period more than half of the time and the Student DOES NOT RECEIVE any child support from their former spouse. This affidavit is to be signed by the Student.

Never Married or Widowed

Use this affidavit when the Student is the sole supporting parent for minor children that live with the Student full-time during the study period and the Student has never been married or is widowed. This affidavit is to be signed by the Student.

Indigenous Identity

Use this affidavit when the Student identifies as Indigenous, but does not have any official supporting documentation confirming the Student’s Indigenous status. This affidavit is to be signed by the Student.

Student With No Income

Use this affidavit when the Student wishes to state that the Student had no income for a specific year. This affidavit is to be signed by the Student.

Student With Non-Taxable Income

Use this affidavit when the Student wishes to state that the Student received non-taxable income during a specific year. This affidavit is to be signed by the Student.

Student With Foreign Income

Use this affidavit when the Student wishes to state that the Student earned foreign income during a specific year but is unable to provide supporting documentation. This affidavit is to be signed by the Student.

Spouse Has No Income

Use this affidavit when OSAP asks the Student for an affidavit from the Student’s spouse stating that the spouse had no income for a specific year. This affidavit is to be signed by the Student’s spouse.

Spouse Has Non-Taxable Income

Use this affidavit when OSAP asks the Student for an affidavit from the Student’s spouse stating that the spouse had non-taxable income for a specific year. This affidavit is to be signed by the Student’s spouse.

Spouse Has Foreign Income

Use this affidavit when OSAP asks the Student for an affidavit from the Student’s spouse stating that the spouse earned foreign income for a specific year but is unable to provide supporting documentation of the foreign income. This affidavit is to be signed by the Student’s spouse.

Parent Has No Income

Use this affidavit when OSAP asks the Student for an affidavit from the Student’s parent stating that the parent had no income for a specific year. This affidavit is to be signed by the Student’s parent.

Parent Has Non-Taxable Income

Use this affidavit when OSAP asks the Student for an affidavit from the Student’s parent stating that the parent had non-taxable income for a specific year. This affidavit is to be signed by the Student’s parent.

Parent Has Foreign Income
