Parent has Foreign Income

Use this affidavit when OSAP asks the Student for an affidavit from the Student’s parent stating that the parent earned foreign income for a specific year but is unable to provide supporting documentation of the foreign income. This affidavit is to be signed by the Student’s parent.

Name of Student’s Parent including middle names (i.e. John Michael Smith)?*

What is the Student’s Parent’s date of birth (i.e. January 1, 1985)?*

What city does the Student’s Parent reside in (i.e. Toronto, Brantford, etc.)?*

What is the applicable tax year in question (i.e. 2023)?*

What was the Student’s Parent’s address during the applicable tax year?* (i.e. 123 Lawrence Avenue, St,. James, Barbados)

How much non-taxable taxable income calculated in Canadian dollars did the Student’s parent earn during the applicable tax year (i.e. $20,000)?*

Explain why you are unable to provide the required documentation proving your income for the year (i.e. my employer did not provide me any documentation and the company no longer exists)?*

What was the source of the non-taxable income (i.e. Working as a computer technician, etc.)?*

How were the Student’s parent’s living expenses met during the applicable tax year (i.e. my income satisfied my living expenses, or family gifts, etc.)?*
