Separated or Divorced with No Child Support

Use this affidavit when the Student and your spouse are separated or divorced and they do not have a formal written separation agreement, divorce decree or court order. The Student’s children live with the Student during the study period more than half of the time and the Student DOES NOT RECEIVE any child support from their former spouse. This affidavit is to be signed by the Student.

What are the names of the Student’s children (i.e. Jill Smith DOB: June 4, 2017, Dave Smith DOB: September 8, 2019 and Evan Smith DOB: June 9, 2021)?*
